Conviva Senior Primary Care announced April 29 it has opened three medical centers next to three Walmart stores in Jacksonville.

They are at 10991 San Jose Blvd., Unit 100; 11900 Atlantic Blvd., Unit 1; and 4250 Philips Highway, Unit 100.

The centers originally opened as Walmart Health clinics. A year ago, Walmart announced it was closing 51 units in Florida, Arkansas, Georgia, Illinois, Missouri and Texas.

Other closed Walmart Health offices in the Jacksonville area include 12100 Lem Turner Road; 7075 Collins Road; 6830 Normandy Blvd.; and 1586 Branan Field Road in Middleburg.

A Conviva news release did not say whether these locations would reopen under their banner.

“At Conviva, we work to help eliminate barriers to care by making a visit to the doctor’s office convenient, and it doesn’t get more convenient than next to a Walmart store,” said Avis Davis, market president for Jacksonville, in the release.

“Like our other centers, these centers are specifically designed with seniors in mind, including dedicated entrances and easy parking. We’re committed to helping our patients lead happier, healthier lives.”

Conviva, a provider of senior-focused primary health care in Florida and Texas, is a wholly owned subsidiary of Humana Inc.

Conviva Senior Primary Care centers are staffed by board-certified physicians, nurse practitioners, medical assistants and operational support staff, the release said.

The centers provide nurse care coordinators, behavioral health specialists, social workers, a clinical pharmacist and referral specialists. Staff is trained to treat the senior population.

In addition to its health care offerings, some Conviva facilities include wellness centers with games, events and activities.

Conviva locations in Jacksonville accept Medicare and a wide range of Medicare Advantage health plans.

Medicare Advantage patients had 278,000 fewer inpatient admissions in 2023 versus those in traditional fee-for-service care models.
